Telehealth Informed Consent

Last updated October 19, 2026

What you are agreeing to

This consent is version 2026-10-16. It is separate from the Patient Terms. California Business and Professions Code section 2290.5 requires the patient to be informed about telehealth and to consent before telehealth is delivered, and requires that consent to be documented. DOC MD documents it by storing this version, the time you accept it, and your account id. A video visit cannot start until that record is stored.

The operator of the software is DOC MD, Inc., a Delaware corporation. The mailing address is 7405 Charmant Dr Unit 2001, San Diego, CA 92122.

Who provides the care

DOC MD, Inc. is a technology platform. It does not practice medicine. The care is provided by the independent treating clinician named when you book. No clinician-patient relationship is formed with DOC MD, Inc. That clinician keeps their own clinical judgment.

What a visit is

A DOC MD visit is live audio and video through Amazon Chime. DOC MD does not save a recording in your account, and the product has no recording switch. You can turn the camera or microphone off. A visit does not include a hands-on physical exam, and some conditions cannot be assessed fully by video. The clinician may tell you to seek in-person care.

Technology can fail. A call can drop, a camera can fail, or a message can be delayed. Those failures are a reason to seek in-person or emergency care when you need it, not a reason to wait.

Emergencies

DOC MD is not an emergency service. It is not monitored for emergency response. Do not use a video visit, the AI tools, messages, or a delayed reply for an emergency. Call 911 or your local emergency number.

AI output supports the treating clinician’s review. It is not emergency triage, it is not a diagnosis, and it is not an independent treatment decision.

Privacy of the visit

Live audio and video are transmitted to connect the call. Notes and prescriptions are stored as the Privacy Policy and the Consumer Health Data Privacy Policy describe. You should take the visit in a private place. Electronic transmission has privacy risks that an in-person visit in a closed room does not.

Your choice

You may refuse telehealth and seek in-person care instead. Refusing or withdrawing this consent does not by itself delete your account. Withdrawal is in Settings on the website and in the app. After you withdraw, a further video visit cannot start until you accept this consent again. The withdrawal time is stored on the consent record.

Accepting this consent means you have read it, you are 18 or older, you understand that the treating clinician is the independent clinician named at booking, and you agree to receive care by video under the limits above.
